Leave Request & FMLA Process

Employee

When an employee will be out for more than 10 consecutive days, they must complete: 1. Leave Request Form

2. FMLA Forms

The Leave Request Form is available in PowerSchool Records, accessible through Quick Links on the ECBOE Homepage.

Principal

After the employee submits the form, it will be automatically routed to the Principal through PowerSchool Records for review

Payroll

For payroll questions about your leave, contact Stephanie Brasher, Payroll Accountant.

  • Payroll will determine: What type of leave days will be used (paid or unpaid), and any salary changes.

Insurance

Employees with insurance or disability-related questions should contact: Alicia Mitchell, Benefits Coordinator

  • Premium amounts, Insurance coverage questions, and Disability issues related to the leave

Personnel

Gina Rich – Handles all remaining personnel information related to the leave

Alice Lee – Handles Highly Qualified (HQ) substitute documentation
