Please read the Medication Guidelines to ensure you understand the rules for administering medication at school.
 
If you must send a prescription or over the counter medication to school you will need to print the School Medication Prescriber/Parent Authorization form and fill it out/have it filled out and bring it to school with the medication. (You must have Adobe 7.1 to download the forms.)
